Q: Can i put the name of the baby on the labels?

I don't see that being possible. The wording is big enough to cover the label and it is everywhere so you see it on all angles of the bottle

Q: will the labels stay on if water bottles are placed in am ice chest?

Yes they will. The label overlaps a little bit if that makes sense. Just as an added precaution put only the bottom of the bottle in the ice.

Q: Do you place the stickers over the original label on the water bottle or peel the original label off?

I peeled off the original. Cut one end and they come right off. They are semi transparent so I think that the old label would show through.
